At Virginia Commonwealth University, we are dedicated to ensuring your voice is acknowledged and respected. Open communication regarding campus incidents, student concerns, and misconduct is essential to promoting student success and maintaining a safe educational environment. This page serves as a central resource for reporting any incidents or issues to the appropriate University officials. If you witness or become aware of something concerning, we encourage you to report it.

The purpose of this form is to gather information about potential violations of the Honor System and Standards of Academic Conduct . This information will be shared with the proper campus offices for appropriate action. So that we may fully investigate your report, please give as much information as you can.

This form may be used to report an accessibility related concern or request regarding programs, services, or activities offered by Virginia Commonwealth University, as well as access to university facilities.

Use this form if you/a student needs support or resources. Your concern will be routed to the Dean of Student Advocacy team to provide assistance.

So that we may fully investigate your report, please give as much information as you can. You can choose to provide your name and contact information or to remain anonymous.

Use this form to report incidents of sexual harassment, sexual assault, dating violence, domestic violence, stalking, sexual exploitation, and related retaliation. VCU will make reasonable efforts to protect privacy while balancing the need to gather information to assess the report and to take steps to eliminate reported misconduct, prevent its recurrence and remedy its effects. While some of the fields below are not required, the university will be better able to assess and respond to complete reports.

Student Conduct Incident Report

CU’s Threat Assessment Team evaluates potentially dangerous situations on campus and responds as appropriate. The team consists of representatives from VCU Police, Human Resources, University Counseling Services, Division of Student Affairs and Enrollment Services, Office of the General Counsel and other units as determined.
Contact the Threat Assessment Team emailing tat@vcu.edu or by calling (804) 828-8940. Callers must provide contact information for a team member to respond. In all instances, report emergencies or dangerous situations immediately to VCU Police at (804) 828-1234.
